Drivers service
What information is required on a log (log book)? Your log must contain the following items, in addition to a graph grid for recording your time:
1. Date
2. Total miles driving today
3. Truck or tractor and trailer number
4. Name of carrier and address
5. Driver’s signature/certification
6. Name of Co-driver (if any)
7. 24-hour period starting time (e.g., midnight, 9AM, noon, 3PM)
8. Remarks
9. Total hours (far right edge of grid)
10. Shipping document number(s) or name of shipper and commodity
Original goes to the Carrier and driver keeps the copy.
Driver is to keep copy of each log record for period of 7/8 days IN YOUR possession. Carriers must keep original copy for 6 months.
Take advantage of combination log and driver’s vehicle inspection report.
Both reports are required and both reports are to be turned in to the Carrier.
